腰椎破壊性脊椎関節症に対して側方進入による低侵襲椎体間固定術で良好な経過を得た2例

　長期透析患者に発症した腰椎破壊性脊椎関節症(destructive spondyloarthropathy：DSA)に対して，側方進入腰椎椎体間固定術(lateral lumbar interbody fusion：LLIF)および経皮的椎弓根スクリュー(percutaneous pedicle screw：PPS)を用いた低侵襲脊椎安定手術(minimally invasive spine stabilization：MISt)を施行し，良好な手術成績を得たので報告する．症例は68歳と66歳の長期透析患者で，頑固な腰痛を主訴としていた．LLIF，PPSを施行したところ周術期合併症は認めず，早期に骨癒合を得ることができ，約1年半の経過でADLの改善も良好である．LLIFおよびPPSを用いたMISt手技は低侵襲で，ハイリスクなDSAに対しても有効な手術オプションになりうる．
