外反母趾に対するDLMO法の成績不良例から学ぶ

　外反母趾に対する最小侵襲遠位中足骨骨切り術DLMO法は，選択的に外側軟部組織解離を加えることで，軽度から中等度のみならず重度の変形に対しても適応可能である．その矯正効果，患者満足度については概ね良好な結果が得られているが，一部の例で外反母趾再発，transfer metatarsalgiaの原因となりえる遠位骨片の背屈変形を生じている．これらへの対策として，手術中，近位骨片遠位端を十分内側に引き出しておく，骨片間の固定性が不十分な場合にはKirschner鋼線を追加するなどの工夫が必要であると考える．
