足関節外果単独骨折の治療経験

背景：足関節外果単独骨折は今後増加が見込まれる．

対象と方法：足関節果部骨折43例44足をAO/OTA(Orthopaedic Trauma Association)分類に沿って分類し，足関節外果単独骨折20例20足の治療方法と短期成績を検討した．

結果：外果単独骨折44-Aを7足認め，2足に手術，5足に保存療法を行った．44-Bを13足に認め，身体所見やグラビティストレステストで三角靱帯損傷が予測された7足と患者希望の1足，計8足に手術，予測されなかった5足に保存療法を行った．JSSFスコアは平均96.3点と良好な臨床成績を得た．

まとめ：外果単独骨折では三角靱帯損傷の有無を判定し，手術適応を決定することが重要と考える．
