診断まで10カ月を要したWartenberg症候群の1例

はじめに：Wartenberg症候群は，橈骨神経浅枝が腕橈骨筋と長橈側手根伸筋の間で前腕筋膜を穿通する部位における慢性絞扼性神経障害である．

症例：55歳女性．先行するde Quervain病があり，症状増悪動作が同じで，感覚障害が乏しかったため，診断が遅れた．手術所見では解剖学的破格はなく，前腕筋膜穿通部での慢性の絞扼が原因と思われた．橈骨神経浅枝の剥離，除圧により症状は改善した．

まとめ：Wartenberg症候群はde Quervain病として治療される可能性が高く，難治性の場合は本疾患を疑う必要がある．
