誌上シンポジウム 腰椎前方アプローチ—その光と影

脊椎前方側方進入手術の低侵襲化・進歩に伴う脊椎疾患に対する治療の進歩と新たな課題へ向けた取り組み—本邦へのLLIF導入後の動向と新たな課題へ向けた取り組みを中心として

　腰椎側方進入椎体間固定術(lumbar lateral interbody fusion：LLIF)の導入に伴い，本邦では腰椎の前方/側方進入手術が行われる頻度が以前よりも増してきている．LLIFの導入に伴って，その周辺機器を利用する形で，また，透視装置を有効に利用することで，通常の腰椎前方側方進入手術に関しても，より低侵襲に，また，より安全性を高める形で行うことが可能になってきている．一方，LLIFの導入に伴って，新たな課題も出てきている．脊椎の前方に主たる病変がある病態に対して，前方支柱の再建を前方/側方進入手術によって直接的に行うことは理にかなっている場合が多い．脊椎前方側方進入手術は古くから行われてきた極めて有効な術式であるが，脊椎前方側方進入手術に関する若手医師などに対する訓練制度に関しては，十分に確立していない部分があった．本稿では，これらの問題に対する対策の一部に関しても紹介する．
