腰椎椎間板由来ガス含有硬膜内腫瘍の1例

　症例は45歳男性，主訴は左下肢痛．MRI，ミエロCT上，L4レベルでガス含有硬膜内腫瘍がみられた．腫瘍は椎間板造影で造影され，ガスは椎間板から特異なルートを経て椎間孔レベルL4神経根分岐部から硬膜内腫瘍内へ流入していた．手術を施行し摘出された腫瘍は，囊腫様でガスが含有されており，病理診断は血管腫であった．硬膜内ガスは椎間板ヘルニアに関連して起こることは報告されているが，本症例では椎間板ヘルニアの所見はなく血管腫が関与していると思われた．本症例は極めて稀な腰椎椎間板由来ガス含有硬膜内腫瘍であった．
