足趾・足関節の関節リウマチ手術の現状と未来

 リウマチの薬物治療が進歩しても,現在のところなくなることのない足部手術において,すべての問題に対処できるように努力し,外科治療を進歩させることがリウマチ関節外科医としての使命である.われわれは関節リウマチ(RA)における足部・足関節部の変形・破壊に対して,関節機能の可及的な温存を目指しながら,機能再建手術を前・中・後足部すべてにおいて行っている.今後,RA症例に対し,さらに脛骨の骨切り手術や足関節鏡手術,distraction arthroplastyなどの関節温存手術の適応が広がることが期待される.
