胸椎硬膜内に発生した血管周皮腫の1例

　症例は50歳，男性．2カ月前からの左下肢のしびれと右下肢の脱力を主訴に当科を受診した．MRIでは，第7胸椎レベルで強く造影される硬膜内髄外腫瘍を認め，脊髄は左側へ強く圧排されていた．Dural tail signを認め，髄膜腫と考え腫瘍摘出術を行った．術後病理診断は血管周皮腫であった．術後に歩行障害は改善した．中枢神経系発生血管周皮腫は主に頭蓋内に発生し，脊柱管内，特に硬膜内発生は極めて稀である．画像上，髄膜腫との鑑別が困難である．治療は全摘出が推奨される．再発率が高く，長期経過後の再発もあり，長期の経過観察が必要である．
