頚椎固定後の嚥下障害とリハビリテーション

はじめに

　頚椎前方固定術(前方固定)や後頭頚椎固定術(O-C固定)といった頚椎固定の手術後に，嚥下障害が稀に合併する．嚥下障害では，食物が通りにくくなる通過障害や気管に入る誤嚥が問題になる．さらに誤嚥性肺炎や栄養障害による体力や意欲の低下のために，日常生活活動や生活の質にも支障を来す．前方固定の場合は，神経障害による機能低下や浮腫による咽頭腔の狭小化が原因で嚥下障害が起こり，リハビリテーションが必要となる¹⁾．一方，O-C固定の場合には，手術による下顎の後退による咽頭腔の狭小化が原因で，手術での対策が重要となる^2,3)．

　そこで頚椎固定術後の嚥下障害へ対応するために知っておきたい嚥下のしくみと評価の方法，および前方固定とO-C固定それぞれの場合の病態と対処法を解説する．
