脊髄神経後枝をターゲットにした新しい神経ブロック—脊椎手術後のマルチモーダル鎮痛の新しい選択肢としての期待

はじめに

　マルチモーダル鎮痛(multimodal analgesia：MMA)は，数種類の非オピオイド薬剤を併用することで相乗効果を引き起こし，優れた鎮痛効果を発揮する．フェンタニルやモルヒネをはじめとするオピオイドは，術後急性疼痛の管理において重要な役割を担っているが，過度の使用は呼吸抑制，悪心・嘔吐，傾眠，排尿障害，イレウスなど様々な副作用を引き起こす．MMAは，オピオイド使用量を削減することができ，オピオイド副作用を軽減する．わが国でも急速に受け入れられ，有効な術後鎮痛法として広がりつつある．

　本稿では，主に脊椎手術に関するMMAを概説するとともに，脊髄神経後枝をターゲットにした新しい神経ブロックを紹介する．
