変形性膝関節症に対するMedial Open Wedge High Tibial Osteotomy

　Open wedge high tibial osteotomyは，変形が比較的軽度な内側型変形性膝関節症および特発性大腿骨内側顆部骨壊死などが適応となる．術後に膝蓋・大腿関節症が進行する例もしばしば報告されるため，手術適応を厳守することが術後のよい成績につながる．手術方法は比較的容易で，感染を除けば重篤な合併症は少ない．外側ヒンジ骨折は術後の後療法において注意を要する合併症である．特にTypeⅠ'，ⅡおよびⅢは骨癒合遷延や偽関節に至る例もある．深腓骨神経麻痺は稀な合併症ではあるが，下腿近位外側の解剖を理解して手術に当たることが必要である．
