　内側型変形性膝関節症に対する膝周囲骨切り術の1つにhybrid CWHTOがある．われわれは，変形解析に基づき，変形中心が脛骨近位にあり膝蓋大腿関節症を伴ったもの，または前十字靱帯損傷後変形性膝関節症に行っている．アプローチは，前脛骨筋の侵襲が少ない直視下法を採用し，骨切りにはボーンソーを用いて，後方と外側の骨皮質の良好な接触が得られることで早期の骨癒合が得られる．また，腓骨の処置は必須で，incurvated flexiated fibular osteotomy(IFFO)を行い良好な骨癒合を得ている．
