—側方から前方法手術のための—後腹膜の解剖

はじめに

　後腹膜の解剖は，腰椎側方椎体間固定術(LLIF)の導入に伴いその注目度が増した．LLIFは低侵襲手術として広がっているが¹⁾，その一方で手術に伴う重篤な合併症の報告もあり，注意を要する．LLIFの主な術中合併症として血管損傷，神経損傷，尿管損傷，腸管損傷，リンパ管損傷などがある^2,3)．合併症を予防するためには，十分な解剖学的な知識を持つこと，前方手術の経験を積むこと，起こりうる合併症を予想すること，手術におけるピットフォールを知ること，術前に各症例の詳細な解剖を検討し解剖の大まかな傾向をつかむこと，それをもとにリスクを評価することが大切である．

　今回は側方から前方法の手術を行うために必要な後腹膜の解剖を解説する．
