MRIによる筋萎縮および筋内脂肪浸潤の定量的評価

 肩腱板断裂の治療において,腱板構成筋の変性が成績不良因子として知られている.一方,元来の定性的評価は再現性が低いと報告されており,また1スライスでの評価は筋退縮により過大評価する可能性が危惧される.そこでわれわれはMRIの3次元2-point Dixonシークエンスを用いて腱板構成筋全体を評価し,筋萎縮と筋内脂肪浸潤を定量的に解析した.高い検者内および検者間信頼性が得られ,解析結果は過去の知見と矛盾しなかった.筋全体の体積および脂肪含有率を正確に評価することにより,各腱板断裂患者の適切な治療方針を判断できるようになることが期待される.
