大腿骨頭軟骨下脆弱性骨折の診断と治療

　股関節における関節内脆弱性骨折として代表的な疾患に“大腿骨頭軟骨下脆弱性骨折(SIF)”がある．本骨折は1999年にYamamotoらがその概念を確立させたが，発症時の単純X線写真では明らかな異常を認めないことが多いため，しばしば診断が遅れる．本疾患の確定診断にはMRI検査が必須であり，その画像所見が特発性大腿骨頭壊死症(ONFH)と類似するため鑑別を要する．SIFの治療の原則は免荷による保存治療となるが，約半数の症例では変形性股関節症へ進行する．本項ではSIFの診断・治療に加えて，SIFの予後予測因子に関しても，いくつかの臨床研究から得られた知見について紹介する．
