　膝関節の特発性骨壊死(spontaneous osteonecrosis：SON)は，部位別に大腿骨内側顆，脛骨内側顆，外側コンパートメントの病変(大腿骨と脛骨を含む)に分けられる．その病態は骨脆弱性を基盤とした軟骨下脆弱性骨折と考えられている．このうち日常診療で遭遇することの多い大腿骨内側SONでは，内側半月板の後角損傷が先行する病変であるとの報告が多い．小走りや階段の踏み外しの際，ブチっとした轢音を膝関節内に感じ，膝窩部を中心に激痛が走り当日は歩行が困難になるなどの症状があれば，MRI検査で後角損傷に特徴的な画像所見を確認し，SONへ伸展することを防ぐために適切な治療をしなければならない．
