肩関節における軟骨下脆弱性骨折

　肩関節における軟骨下脆弱性骨折(subchondral insufficiency fracture：SIF)は，股関節SIFと同様に骨粗鬆症を有する高齢女性にみられるが，その報告は極めて少ない．急速な関節裂隙の狭小化を伴いながら破壊が進行する例もあるため，SIFを疑った場合は，速やかにMRI検査を行うことが望ましい．腱板断裂合併例では，骨頭圧潰に伴う関節不安定性によって腱板断裂サイズが拡大する可能性が指摘されている．病理組織像では，層状構造の骨壊死巣はなく，旺盛な仮骨や肉芽組織がみられる．外傷歴がないか，あっても軽微な高齢女性の肩関節痛患者の診療に際して，SIFは考慮すべき重要な疾患の1つである．
