頚椎OPLLに対する手術—後方法の立場から

　後方法で対処可能な頚椎OPLLは，K-line(＋)：椎弓形成術で十分除圧が得られる，K-line(＋)・OPLL最大圧迫高位の椎間可動域大：椎弓形成術の成績が劣り後方固定追加を要する，K-line(−)・頚椎アライメント前弯：後方除圧固定術で良好な成績が得られる，などである．一方，後方法で対処困難なOPLLは，K-line(−)・アライメント後弯：後方除圧固定術では対処しきれず，前方除圧固定術の適応，頚椎矢状面アライメント不良例：固定追加しても矢状面アライメント悪化は防止できない，などである．
