　頚椎後縦靱帯骨化症(ossification of the posterior longitudinal ligament：頚椎OPLL)に対する前方法(前方除圧固定術)は，脊髄・神経根の解剖学的な位置や形態を取り戻し，固定によって脊髄にとって安定的な環境を構築することにより，良好な成績が期待できる優れた手技である．骨化巣開削を適切かつ安全に行うためにはアプローチや椎体展開の習熟が不可欠である．また，骨化占拠率の高い症例には硬膜欠損がみられるため，術中のくも膜温存への配慮や術後の髄液漏に対する対処が必要となる．
