頚椎後弯変形に対する矯正固定術—前・後合併手術はどういう時に必要か？

文献概要

　頚椎後弯変形に対する固定アンカーとしては，固定力に優れる頚椎椎弓根スクリューが適している．頚椎後弯変形が非整復性の場合，前方解離や椎間関節切除が必要である．脊柱管矯正を合併している症例では，後弯を整復する前に必ず椎弓形成術などの後方除圧を行ってから後弯を矯正することにより，医原性の脊髄損傷を回避できる．また，術前C4/5椎間孔狭窄合併例では，予防的椎間孔拡大術を併用しておく．治療アルゴリズムに則って術式を決定することで，本疾患に対して良好な成績を得た．
