胸腰椎移行部破裂骨折に対するligamentotaxisを用いた後方固定術

　胸腰椎移行部破裂骨折に対するligamentotaxisを用いた後方矯正固定術は現在広く行われている．これは，骨折した椎体の上下隣接椎体へ椎弓根スクリューを挿入して，骨折により生じた後弯変形を整復し，また，頭尾側へ牽引することにより前縦靱帯，後縦靱帯の緊張を利用して圧潰している椎体の形状を整復するものである．良好な整復が得られる一方で後方のみで手術を終えるので，前方法，前方後方法に比べて低侵襲な手技と言える．本稿ではその原理，留意すべき点などにつき概説する．
