成人脊柱変形に対する後方矯正固定術—上下関節突起切除と後方椎体間固定による矯正

　成人脊柱変形手術の目標は良好な全脊椎アライメントを獲得することである．特に矢状面においてはL4-S1に大きな前弯を獲得することが重要である．L4/5とL5/S1に上下関節突起の切除と後方椎体間固定を行いL4を水平化したのち，尾側から鉛直線に平行に立ち上げたロッドにL3より頭側の椎体をtranslationしていくことにより，矢状面，冠状面ともに良好な矯正が得られる．
