小児期の股関節手術既往患者に対する人工股関節全置換術の成績

背景：発育性股関節形成不全(developmental dysplasia of the hip：DDH)に対して小児期に骨切り術などが施行され，後に末期股関節症となり人工股関節全置換術(total hip arthroplasty：THA)に至ることがあるが，その成績の報告は少ない．

対象と方法：当院で行ったDDH由来のTHA2,606関節を対象とし，小児期の手術既往のある群と手術既往のない群で比較・検討した．

結果：手術既往あり群は有意に若年で，手術時間は長く，出血量は多く，術後成績はやや劣る結果であった．

まとめ：小児期股関節手術の既往のある症例がTHAを要する場合，比較的若年での手技的に困難なTHAとなり，術後成績も劣る傾向になる．
