【総論】インプラント周囲の非定型大腿骨骨折および類似した骨折の診断と治療

　インプラント周囲骨折のうち，治療に難渋する横・斜骨折と非定型大腿骨骨折(atypical femoral fracture：AFF)の関連が注目されている．Vancouver type B1，AO分類32A2，A3(斜骨折，横骨折)の9骨折に関してAFFの診断項目，大腿骨の外弯，骨癒合期間，偽関節の発生に関して検討した．全例AFFの診断基準を満たし，ビスフォスフォネート製剤，大腿骨外弯の影響も示唆された．治療はプレートによる骨接合術を選択する場合，外側ロッキングプレート単独でgapを残した場合には遷延癒合となる可能性が高いため，骨片間圧迫をかけたうえでの前方と側方へのdouble platingが有用であると考えられた．
