【THA】THA後の骨盤側のインプラント周囲骨折の問題点と注意点

　人工股関節置換術後の骨盤側のインプラント周囲骨折は，大腿骨側と比較するとまれではあるが，その診断と治療に関しては注意が必要である．転位のほとんどない例には保存療法が可能であるが，転位の大きな例，骨盤輪の破綻を伴う例，もともとインプラントのゆるみを生じていた例などには，骨接合術や人工股関節再置換術を要する．骨粗鬆症をベースとする脆弱性骨折が関連している場合があり，術後には骨粗鬆症の積極的な治療が必要と考える．
