【THA】大腿骨ステムコンセプトの違いによるセメントステム周囲骨折の頻度と治療

　セメントステムはセメントレスステムに比して，インプラント周囲骨折が少ないとされるが，同じセメントステムでもtaper polished型ステムはcomposite beam型ステムより骨折リスクが高いので，インプラント選択の際には年齢や骨脆弱性も考慮に入れる必要がある．一方，インプラント周囲骨折の治療に際してはVancouver分類B1を含むBの症例に対する骨接合術の成績は不良だった．特にtaper polished型ステムはコンセプト上ステムがセメントに固定されていないためと推測される．Vancouver分類Bに相当するセメントステムのインプラント周囲骨折に対しては再置換を第一選択として術前計画を立てたほうがよい．
