【THA】大腿骨ステム周囲のoccult fractureの診断と治療

　初回および再置換の人工股関節全置換術(total hip arthroplasty：THA)248例276股の術後大腿骨のステム周囲の不顕性骨折(occult fracture)を調査し，有病率と骨折の傾向から本骨折の診断と治療について検討した．初回THAでは8股(3％)，再置換(THA)では1股(11％)に骨折を認めた．術中の骨折の確認は困難なため，術後早期にCTを施行するべきである．ステムの種類や術前の骨質，髄腔形状により骨折の頻度は変化し，骨折の部位や長さによって経過も変化すると考えられ，今後も調査検討が必要である．
