【コラム】大腿骨ステム周囲骨折の骨接合のテクニック

　大腿骨ステム周囲骨折は，大腿骨内にステムが挿入されていることで骨接合術の方法を個々の症例に応じて変える必要があり，最も治療の難しい骨折の1つである．骨接合術においてVancouver分類とAO/OTA分類の骨折形態を把握することは治療方針を立てる上で有用であり，それぞれの骨折形態に合った固定方法について習熟することが必要である．今回，大腿骨ステム周囲骨折に対する骨接合術について基本的な考え方と手術手技について述べ，そのピットフォールについて解説する．
