【TSA】人工肩関節置換術(TSA)周囲骨折の診断と治療

　Total shoulder arthroplasty(TSA)ステム周囲骨折の手術治療を行った1例を経験したため具体的な術式を含め報告した．78歳女性．転倒で左上肢を受傷した．右リバース型人工肩関節置換術(reverse shoulder arthroplasty：RSA)の上腕骨のステム下骨折を認めた．プレート固定に加えて柱状同種骨移植(strut allograft)を併用した2面での固定を施行した．3カ月で肩関節の機能はほぼ術前まで回復し，術後1年で完全な癒合を得た．Strut allograftの併用は生物学的に骨癒合に有利なだけでなく，骨折部以遠の上肢の重量が回旋力として骨折部にかかることを考慮すると構造的にも有利と考えられる．
