脊椎転移診療における放射線治療のリスク

　放射線療法は転移性脊椎腫瘍に対する主要な治療であるが長期の局所制御は難しいことが多い．そのため長期生存例では放射線治療後に腫瘍が再燃し，手術が必要となることがある．放射線照射は皮膚・軟部組織，骨，髄膜などに悪影響を及ぼし，照射後の手術では創トラブルや感染，骨癒合不全，髄液漏や硬膜への腫瘍浸潤などの合併症リスクが高まる．照射後に手術が必要となる状況を極力回避することが重要であり，多職種で協議の上，慎重に治療方針を決定する必要がある．照射後の手術が必要な場合は，各種合併症に可能な限りの配慮・対策を講じて臨むことが肝要である．
