“Floating” Elbow/Kneeに対するLimb Damage Control

　Floating elbowもfloating kneeもfloating segment(FS)により患肢が非常に不安定な状態となる．両者とも開放骨折やそれに伴う軟部組織損傷，神経血管損傷など局所ダメージ・コントロールの観点から創外固定(external fixator：EF)が有用である．特にfloating kneeにおいては深部静脈血栓症やそれに伴う肺塞栓症，急性肺障害など全身的観点でも創外固定は必須の手段である．創外固定の固定力を高めるためにはFSにもハーフピンを挿入することになるが，神経血管束の走行には細心の注意を払う必要がある．
