周術期の抗血小板薬中断により左冠動脈主幹部に超遅発性ステント血栓症を生じた1例

　心筋梗塞に対する薬剤溶出性ステント(drug-eluting stent：DES)を用いた経皮的冠動脈形成術(percutaneous coronary intervention：PCI)後9年で施行した整形外科手術直後に超遅発性ステント血栓症を生じた症例を経験した．PCIでは第1世代DESが使用されており，手術前の抗血小板薬の休薬が発症の誘因となった可能性が高いと考えられた．DESを使用したPCI後では，長期間経過した症例であっても周術期の抗血小板薬の取り扱いに関して十分に注意する必要がある．
