成人脊柱変形　どのような変形にどのような椎体骨切りを行うべきか？

　成人脊柱変形患者に対する矯正固定手術の術式選択基準，LLIFとPSOの使い分け，特に骨性癒合を伴った硬い後側弯変形に対するPSOの適応，また，PSO施行時の骨切り高位選択基準，固定範囲，2期的手術計画の適応について，当科で使用している腰椎前弯矯正目標と術式選択ストラテジーを用いて説明する．また，矯正固定におけるoblique take offのリスクと複数椎の骨切りを要する症例について，われわれが経験し，治療に難渋した症例をもとに説明する．
