成人脊柱変形に対する前方固定術の意義と適応選択の重要性

　近年増加しつつある成人脊柱変形症例では，各種検査で主訴に直結する所見以外にも多くの変性所見，狭窄所見を認めることが多い．われわれ脊椎外科医はそのすべてに処置を施すのか，もしくは必要最低限の処置を行うのか，患者のニーズも踏まえながら熟考して治療方針を検討すべきである．その上で脊柱変形の矯正には理想となるパラメータを念頭に置きつつ，状況と必要に応じた前方・後方手術の適応と実施を検討していく必要がある．
