放射線照射長期経過後に生じ治療に難渋した大腿骨転子下偽関節の1例

　放射線照射長期経過後，同部位に生じた大腿骨転子下骨折偽関節のため治療に難渋した症例を経験した．54歳女性．27歳時に右大腿近位部脂肪肉腫に対して腫瘍摘出術，術後放射線治療(60Gy)を行った．51歳時に転倒し，右大腿骨転子下骨折の診断で，他院で2度の骨接合術を施行したが骨癒合せず，初回受傷後3年でインプラント折損を来たしたため当院に紹介された．当院でも2度の手術を行ったのち，最終手術後2年で骨癒合を得た．大量放射線曝露後の骨に対する晩期障害は恒久的で，偽関節となる可能性があり，放射線照射の既往の聴取と，手術方法に十分な注意を要する．
