ピロン骨折を見逃さないための病歴聴取

病歴聴取：診断は，詳細な病歴聴取と身体所見の評価から開始する．受傷機転から外力の大きさとそれに伴う骨および周囲軟部組織へのダメージを推測することができる．加えて，基礎疾患や喫煙習慣の聴取は，その後の治療戦略に影響する可能性がある．開放骨折では，受傷場所も必ず聴取する．汚染の種類や程度が異なると，後の抗菌薬投与に大きく関係するためである．

受傷機転：若年者の本外傷の受傷機転の多くは，高所転落や交通外傷などの高エネルギー外傷である．一方，高齢者では，転倒や軽微な転落によっても関節面の粉砕を伴うことがある．外観上は，短縮，回旋，角状変形のどのような変形も来しうるため，果部骨折との鑑別は困難であり，受傷機転が本骨折を疑うもっとも有用な情報となる．
