距骨骨軟骨病変に対し関節鏡視下に骨接合術を施行した2例

　距骨骨軟骨病変(osteochondral lesion of the talus)に対して，関節鏡視下に骨接合術(arthroscopic reduction and internal fixation)を施行した2例を報告する．症例1は15歳男性，距骨滑車前外側に骨軟骨骨折を認め，前外側ポータルより吸収性ピンで固定した．症例2は15歳女性，陳旧性の距骨滑車内側の骨軟骨病変に対して，脛骨内果に5mm径の骨孔を作製し，そこから吸収性ピンで固定した．従来の治療として，骨片が小さければ骨穿孔術が行われてきたが関節面の整合性・軟骨の温存という観点からも可能な限り骨接合を行うことが望ましいと考える．本術式は関節鏡により低侵襲に行うことが可能であり，有用である．
