足関節後果骨折に対する新たな手術手技—鏡視下Handlebar法

足関節後果骨折を鏡視下に整復する鏡視下handlebar法について詳述する．対象はHaraguchi type 1または2の後果骨折で，Kirschner鋼線を後果骨片に内外側に貫通できる骨折型である．手術では腓骨骨折の整復固定後に後果骨片にKirschner鋼線を貫通させ，これを自転車のハンドル(handlebar)のように操作して鏡視下に後果関節面を整復し，前方から数本の裸子で固定する．本法は手術手技が比較的簡便で，関節内全域を観察しながら骨折整復を行うことが可能であり，これまで足関節後果骨折治療が内包してきた多くの障壁を解決できる可能性がある．
