粉砕型距骨骨折に対し初期治療として人工距骨置換術で対応した治療成績

粉砕型距骨骨折症例に対し初期治療として人工距骨置換術を適応し，その治療成績を報告する．対象は6例で，平均年齢は40.3(19〜59)歳であった．術後治療成績はJSSF scale，SAFE-Q，関節可動域を計測した．平均経過観察期間は61(12〜115)カ月であり，置換術後の治療成績はおおむね良好であった．3例は距骨圧潰が強く，手術前に従来の距骨容積を維持する目的で創外固定器を装着した．粉砕型距骨骨折は整復手技の困難さ，高頻度の無腐性骨壊死の発生や変形性関節症への進行などで治療に難渋するため，人工距骨も初回治療として適応になり得る．
