腰部脊柱管狭窄症手術後の腰痛に伴う機能障害残存の予測因子

目的:腰部脊柱管狭窄症術後の腰痛に伴う機能障害残存に対する予測因子を明らかにすること.方法:固定術50例と除圧術61例の計111例(平均年齢70.8歳)を対象に,腰痛に伴う機能障害の指標としてRDQを術前と術後6カ月に評価した.機能障害の残存(RDQ≥1)を目的変数としたロジスティックス回帰分析を行った.結果:RDQは術前7.9±4.7,術後6カ月1.9±2.7であった.腰痛に伴う機能障害残存の独立した予測因子は術前JOABPEQ心理障害得点であった.まとめ:腰痛に伴う機能障害は全般的には手術により改善するが,腰痛に伴う機能障害残存の予測因子は,術前の心理的要因であった.
