上腕二頭筋麻痺で肘屈曲がMMT4である症例

背景：肘屈曲に必ずしも上腕二頭筋の収縮を要しない症例を経験した．症例：22歳男性．交通事故により上腕骨骨折等を受傷し，骨接合術後1カ月で，肘屈曲時に上腕二頭筋の収縮は認められなかったが，muscle manual testing(MMT)4であった．電気生理学的検査で，筋皮神経のうち上腕二頭筋内側頭の分枝に神経断裂(neurotmesis)が認められ，MRI検査でも，二頭筋の萎縮がみられた．まとめ：肘屈曲は，腕橈骨筋などにより代償されるため，筋力評価をする場合，上腕二頭筋の筋収縮を確認して筋皮神経損傷を見逃さないよう注意を要する．
