大腿骨転子間骨折(AO分類31A3)・逆斜骨折(Reverse Obliqueタイプ)に対する治療法

大腿骨転子間骨折(AO分類31A3)や逆斜骨折(reverse obliqueタイプ)の定義を明らかにし，二村分類をもとにインプラント選択も含めた治療戦略を提唱する．TypeⅠ(lateral wall pattern：LW)は通常の転子部骨折の治療戦略に準じる．TypeⅡ(transverse pattern：TV)は整復位獲得と保持が難しい横骨折であるが，適切なショートネイル(short femoral nail：SFN)で対応可能である．Type Ⅲ(reverse oblique pattern：RO)は真の逆斜骨折で，鉗子固定やワイヤリングにより外側壁と骨幹部を一体化させたうえでミドルネイル以上の長さの髄内釘で固定する．
