転子部骨折術後合併症に対する治療—カットアウト・偽関節・感染のサルベージ

大腿骨転子部骨折治療の代表的な術後合併症である①カットアウト，②偽関節，③感染に対する処置や救済手術について述べる．偽関節とカットアウトはそれぞれ関連しており，ラグスクリューやブレード周囲を中心とした骨頭内や大腿骨近位部の骨欠損を詳細に評価して治療計画を立案・遂行する必要がある．多くの症例で高度な人工物置換術が適応となるが，偽関節例では再骨接合が適応となる症例もあり，その手技にも精通する必要がある．感染に対しては躊躇せず検体の採取と洗浄，デブリドマン，抗菌薬局所投与(抗菌薬含有セメントビーズ留置やCLAPなど)を行い，安定したインプラントであればまず温存を試みる．
