巨大骨欠損を伴う症例への対応

人工股関節置換術(THA)および人工骨頭置換術後の感染(PJI)は比較的稀な合併症ではあるが，症例によっては診断に難渋するため，治療方針の決定は慎重に行う必要がある．感染を鎮静化するために繰り返されたデブリドマンにより，寛骨臼や大腿骨近位に骨欠損が生じる．ましてや，骨髄炎と判断され大腿骨近位が大きく切除・摘出された場合，次に行う再建が難しくなる．また，感染の診断がなされず経過とともに骨融解が進んでしまい，巨大な骨欠損が生じてしまう場合もある．まずは感染と診断したら，感染の鎮静化を試み，感染が鎮静化したことを確認したうえで，再建が必要になる．巨大な骨欠損を生じてしまった場合には，骨バンクから供給された安全で良質な同種骨による欠損部の補填が有用と考える．
