体軸性脊椎関節炎の診断と治療

脊椎関節炎(spondyloarthritis：SpA)は非対称性末梢関節炎，付着部炎，炎症性腰背部痛，関節外症状を共通の臨床症状とする種々の疾患からなる総称である(図1)1)．これまで歴史的に疾患概念の変化に伴い表現や含まれる疾患が変化してきており，以前は血清反応陰性脊椎関節症(seronegative spondyloarthropthies：SNSA)と呼ばれていた．2009〜2011年にAssessment of SpondyloArthritis international Society：ASAS)より体軸性脊椎関節炎(axial SpA：ax SpA)(図2)2)および末梢性脊椎関節炎(peripehral SpA：pSpA)の分類基準が示され現在に至っている．
体軸性脊椎関節炎には仙腸関節のX線基準［強直性脊椎炎(ankylosing spondylitis：AS)のために策定された改訂New York基準3)］を満たす体軸性脊椎関節炎(radiographic ax SpA：r-ax SpA)と同基準を満たさない体軸性脊椎関節炎［non radiographic(nr)-ax SpA］に大別され，r-ax SpAは改訂NY基準で定義されるASと90％以上の相同性を示し同一疾患として捉えられている．したがってnr-ax SpAはax SpAからASを除いたものと理解される．広義では乾癬性関節炎(psoriatic arthritis：PsA)や炎症性腸疾患(inflammatory bowel disease：IBD)に伴う脊椎関節炎などの体軸罹患もax SpAには含まれることから，nr-ax SpAを診断名にすると複数のSpAの診断重複となり混乱が生じるため，近年ヨーロッパを中心にASやnr-ax SpAを診断名にせず，ax SpAのみを診断名にすることが提唱されている4)．一方，本邦ではASが2015年に難病に指定され，少なくとも現時点では明確にnr-ax SpAとASを区別して診断する必要がある．
