骨幹端掌側皮質破壊を伴うAO分類type C3橈骨遠位端骨折に対する治療戦略—掌側皮質連続性獲得の有用性について

骨幹端掌側皮質破壊を伴うAO分類type C3橈骨遠位端骨折8例を対象とした．まず創外固定牽引による骨長維持を行い，ハンド用ミニプレートにより掌側皮質を整復固定して連続性の回復と安定化を獲得した．そのうえで掌側部をヒンジや壁として用いて，各種手法を用いて関節内外の整復操作を施行した．全例において，術後の関節外アライメントは正常範囲内，関節内step-offは1mm以内であった．最終観察時，Mayo Wrist Scoreは平均85点で，全例で骨癒合し，矯正損失は認めなかった．同骨折に対する同手術法は有効な戦略になり得る．
