今後のリウマチ診療において，整形外科医がどのように関わっていくか—リウマチ外科医の立場より

関節リウマチ(RA)に対する手術は近年減少傾向にあるとの報告が多い．われわれの施設では手術件数は年間80〜100件で推移しているが，その内訳は膝や股関節などの大関節手術が減少し，足趾や手指などの小関節手術の割合が増加している．生物学的製剤時代の昨今，RA薬物治療に関わる整形外科医は減少していると思われるが，適切なタイミングで手術を勧めることができるのは整形外科医によるRA診療の利点である．また内科医や開業医で薬物療法を行っている症例でも，関節注射や定期的な関節評価などにRA整形外科医が介入し，治療困難症例を少しでも減らせるような連携が今後ますます重要である．
