骨盤輪損傷の初期治療，初期固定

骨盤輪損傷は骨折治療のみならず，ときに集学的治療を必要とする．特に出血性ショックを伴う場合，プロトコルに沿った外傷蘇生と止血操作を行う必要がある．止血のための治療介入には，経カテーテル的動脈塞栓術(TAE)やpelvic packing，創外固定などが挙げられるが，優劣ではなく相補的に行う必要がある．確定的内固定のタイミングは議論の余地があるが，近年ではearly appropriate careの概念に沿って早期内固定を行うようになってきている．初期治療から内固定まで，施設の人的，物的資源に大きく左右されるため，各施設で適した治療戦略を練る必要がある．
