大腿骨顆部脆弱性粉砕骨折に対して一期的人工膝関節置換術を施行した1例

背景：骨粗鬆症や変形性膝関節症(膝OA)を有する，高齢者の大腿骨顆部脆弱性粉砕骨折に対する観血的整復固定術(ORIF)は，強固な固定や早期歩行を得ることが難しい．症例：78歳女性．膝OAと骨粗鬆症を伴うAO分類33C3.1の骨折に対して受傷後10日に腫瘍用人工膝関節置換術(TKA)を施行し，術後10日で歩行器歩行訓練を開始した．術後2年6カ月時点で，関節可動域は0〜100°，杖歩行可能であった．まとめ：高齢者の大腿骨顆部脆弱性粉砕骨折に対する一期的TKAは早期歩行が得られ，高齢化の進む現代社会においては，介護予防に有用な治療選択肢の1つと考えられる．
