洗浄液周期的自動注入機能付き陰圧閉鎖療法(NPWTi-d)による植皮の固定

はじめに

　植皮の固定として近年，陰圧閉鎖療法(negative pressure wound therapy：NPWT)が広く使用されており^1，2)，従来のタイオーバー法と比較して合併症も少なく植皮生着がよいとの報告もある²⁾．一方，NPWTに自動洗浄機能を付加したNPWTi-d(negative pressure wound therapy with instillation and dwelling)による植皮の固定は，植皮の浸軟や浮きが懸念されることからほとんど用いられていない³⁾．感染部位に植皮は生着しないため，汚染創に対する植皮を行う際には慎重になる必要がある．われわれは特に感染が懸念される症例に対する植皮の固定に，当院医療安全委員会の承認を得てNPWTi-dを使用しているが，合併症なく良好な成績を得ている⁴⁾．
